This patient was concerned about under-eye bags, dark circles, and a hollowed appearance beneath the eyes. These changes were due to fat herniation over the orbital septum, tear trough deformity, and midface volume loss—an anatomic condition she was born with that tends to worsen over time.
To address this, she underwent a Transconjunctival Lower Blepharoplasty, a technique performed without any external skin incisions, combined with orbital retaining ligament release and fat grafting to the tear troughs and cheeks using buccal fat harvest. This comprehensive approach smoothed the under-eye area, softened the tear troughs, and restored natural, youthful volume to the midface.

Our vibrant 67-year-old patient , was unhappy with the appearance of her eyes. Excess skin on both her upper and lower eyelids, particularly under the eyes where she had developed malar bags or festoons, was a constant bother. She found her eyes often appeared closed, leading her to rely on sunglasses to minimize squinting. To address her concerns, we performed a comprehensive surgical procedure. This included direct excision upper and lower blepharoplasty to remove the excess skin. We also focused on releasing the orbitomalar ligament and zygomaticocutaneous ligament, which contributed to the presence of malar bags or festoons. Additionally, we performed ptosis correction by shortening Muller’s muscle and conjunctiva, enlarging the aperture of the eye. The results speak for themselves!
